Is my doctor or hospital PPO?
Your insurance card has the name and phone number for your PPO Network. You can contact them by phone or visit our Find a Provider page and select your network to visit the PPO website.


Did my claim get paid?
Please visit the member section of this website and register to view your claim status.


What is my ID number?
Your ID number can be found on the front of your insurance card above your name.


What if my doctor/hospital won’t accept my insurance card?
Ask the doctor/hospital to contact us at (208)343-2964. Many times this issue can be resolved between us and the provider. If they refuse to contact us please follow these steps:

Obtain an itemized billing from your doctor/hospital including diagnosis, services performed and charges for each of those services, the doctor/hospital name and address, the date of service and the patient name. If you paid the bill in full, make sure the doctor/hospital indicates that on the bill. Send a copy of this to us at PO Box 45073 Boise, ID 83711. Include a copy of your ID card for identification. We will reimburse you.


How do I get reimbursed for a doctor visit that I had to pay for?

What is covered under my flexible spending account (FSA)?

What documentation do I need to submit a flex claim?
To submit a FSA claim, you need to have 1. A flex claim form found here and 2. Proof that the expense you are claiming was performed/received. The easiest form of proof is a copy of the Explanation of Benefits for medical, dental and vision services. Cash register receipts can be sent for over the counter medication or prescriptions. Please keep a copy of all flex claims submitted for your own records.


What if I don’t use all the money in my flex account?
The FSA program is a “use it or lose it” program. This means if you do not file valid claims for the money in your account at the end of the FSA year, then you lose the money. The IRS does allow 90 days after the end of a FSA plan year for you to file any remaining charges from the previous FSA plan year.

How do I submit a claim for a prescription that I paid for out of my pocket?
Mail a copy of your itemized prescription slip to us at PO Box 45073 Boise, ID 83711. Include a copy of your ID card for identification.

How do I change or update my address?
Please register to login to the member section of our website and send a request to us asking that it be changed.

The MSA Ehealth website uses secure sockets layer (SSL) encryption technology, the industry standard. Regardless of whether you see the alert box, your information is still protected by SSL.
